WHY NOT JUST PAY A LARGER BONUS?
A Bigger Signing Bonus Isn't the Only Answer.
When a valued physician is being recruited, the instinct is often to simply offer more money upfront. A signing bonus can help close an offer, but it is designed for a different purpose. A structured retention incentive is designed around a different question: how do we keep this person here, year after year?
| Aspect | Traditional Signing Bonus | Structured Retention Incentive |
|---|---|---|
| Primary purpose | Encourage a physician to accept an offer | Encourage continued service |
| Payment timing | Often upfront | May be linked to future conditions |
| Long-term incentive | Depends on contract terms | Can be structured around milestones |
| Flexibility | Negotiated at hiring | Customized arrangement |
| Administration | Typically simpler | Potentially more complex |
| Legal / tax review | Employment and tax considerations | Potentially additional deferred compensation requirements |
A retention program is not automatically less expensive than a bonus. Its potential value lies in the structure and timing of the incentives — rewarding the people who stay, rather than paying upfront for a commitment that may not last.
Important: Both approaches involve employment, tax, and — for structured incentives — potentially deferred compensation considerations. Every arrangement requires individualized legal and tax review before implementation.
ONE ILLUSTRATIVE RECRUITING SCENARIO
An Offer Built Around a Longer-Term Relationship.
Hypothetical example only — not a client engagement, recommendation, or prediction of results.
A physician-owned practice is recruiting a specialist.
The practice wants to strengthen its offer while keeping base salary aligned with its compensation framework. The candidate is also considering another medical group.
Instead of relying solely on a larger signing bonus, the practice could explore an additional incentive earned through agreed-upon future service milestones.
- 01
Evaluate the offer
Review the role, compensation, budget, and candidate’s priorities. Determine whether the proposed relationship is W-2 employment or an eligible 1099 arrangement.
- 02
Define future milestones
Model potential incentives linked to continued service at agreed intervals, with clear earning conditions, payment timing, and departure provisions in a written agreement.
- 03
Review before committing
Coordinate individualized legal and tax review, confirm the practice’s funding obligations, and establish administration and reporting before presenting final terms.
The potential value is in the structure and timing of the incentive. It is not automatically less expensive than a bonus, does not replace competitive compensation, and cannot guarantee acceptance or continued service.
W-2 employee benefits and 1099 independent-contractor arrangements require separate evaluation. Eligibility, worker classification, physician compensation rules, and any deferred compensation requirements depend on the circumstances. Qualified legal and tax professionals must review the arrangement. Insurance is optional, not required.
